At Southwest Florida Dental Group, our office policy has always been to work with every patient, and to provide a warm and welcoming environment where they will receive the highest quality of dental care.


Once you make an appointment at our office, that time is exclusively reserved for you. If, for some reason, you are no longer able to make your appointment, please call our office with 24-hours notice to reschedule.


If you have dental insurance, please bring your insurance card and photo ID with you to your appointment. Our office staff will file any claims that need to be made, and work to get you the maximum amount of benefits allowed by your insurance plan. At Southwest Florida Dental Group, we are happy to accommodate you with any billing or insurance question you may have. Call us today!


Payment is expected at the time services are rendered, unless other arrangements have been made. This includes any co-payment or deductibles (balances not paid by your insurance).

For your convenience, we accept cash, checks, Visa, MasterCard. American Express and Discover. We do accept some insurance, but are always happy to process your claims for your convenience.


Below you will find our new patient forms. Feel free to print and fill them out before your first appointment with us. If you choose to fill out the forms at our office, please arrive 15 minutes early to do so.
